Statins and Bodybuilders: What You Need to Know About Cholesterol, Muscle Health, and Performance 

Statins are among the most commonly prescribed medications in the world for lowering cholesterol and reducing cardiovascular risk. But in the bodybuilding world—where performance, muscle integrity, and aesthetics are critical—many athletes are understandably hesitant about taking any medication that might interfere with their training or recovery. 

As a doctor working closely with bodybuilders, I often get questions like: 

“Will statins affect my strength?” 

“Do I really need them if I’m lean and muscular?” 

Let’s unpack the science and myths behind statins and what you should consider. 

What Are Statins? 

Statins (e.g. atorvastatin, rosuvastatin, simvastatin) are a class of drugs that lower LDL cholesterol (“bad cholesterol”) by blocking HMG-CoA reductase, an enzyme involved in cholesterol production in the liver. They also have anti-inflammatory and plaque-stabilising effects that help reduce the risk of heart attacks and strokes. 

Why Would a Bodybuilder Need a Statin? 

It’s a common misconception that cardiovascular disease only affects the overweight or sedentary. In reality, bodybuilders may have unique risk factors, such as: 

  1. Anabolic Steroid Use
  • Increases LDL and lowers HDL 
  • Promotes arterial stiffness and plaque formation 
  • Can raise blood pressure and left ventricular hypertrophy 
  1. High-Protein, High-Fat Diets
  • Some bodybuilding diets include high levels of saturated fats or cholesterol, especially during bulking phases 
  1. Genetic Dyslipidaemia
  • Some athletes may have familial hypercholesterolaemia and not realise it until screened 
  1. Low Estrogen Levels in Men
  • From AAS or post-cycle therapy: estrogen helps maintain favourable lipid profiles 

Key Clinical Features Leading to Statin Prescription 

  • Elevated LDL cholesterol or total cholesterol 
  • Low HDL cholesterol 
  • High triglycerides 
  • History of cardiac event or strong family history 
  • Coronary artery calcium score >100 
  • Use of AAS with significant lipid derangement 

Common Concerns Among Athletes: Are They Justified? 

💪 Will Statins Affect Muscle Gains? 

  • The majority of users experience no impact on muscle strength or size 
  • However, up to 5–10% report muscle symptoms: soreness, cramping, or fatigue 
  • These symptoms are often dose-dependent and may improve with: 
    • Switching to a different statin (e.g. from simvastatin to rosuvastatin) 
    • Using alternate-day dosing 
    • Taking Coenzyme Q10 (CoQ10) supplements (controversial but low-risk) 

🩸 What About Liver or Kidney Damage? 

  • Liver enzyme elevations can occur but are rarely clinically significant 
  • Regular monitoring (ALT, AST, CK) is advised, especially in: 
    • High-dose users 
    • Those using other hepatotoxic substances (e.g. AAS, SARMs) 

 Fatigue and Recovery 

  • Some athletes report mild reductions in exercise tolerance, though this is uncommon 
  • Careful adjustment of the type and dose of statin usually solves the issue 

Benefits That Matter for Bodybuilders 

You train to build muscle, stay lean, and stay in peak shape—but none of that matters if your heart or arteries are silently deteriorating. 

Why statins can be worth it: 

  • Reduces long-term cardiovascular risk 
  • Mitigates damage from AAS use 
  • Stabilises plaque: Helps prevent sudden cardiac events 
  • Can improve endothelial function and reduce vascular inflammation 

In high-risk individuals, statins can preserve long-term performance and lifespan, not take away from it. 

Impact on Lifestyle, Work, and Competition 

Driving & Employment 

  • No restrictions with statins per se 
  • However, statin use may imply underlying cardiovascular risk—important in jobs with medical assessments (e.g. military, pilots, commercial drivers) 

Competitive Sports 

  • Statins are not banned by WADA 
  • Athletes must declare all medications during competition 
  • Consider potential for cramping or performance dips during early treatment (monitor closely) 

Alternatives and Adjuncts 

In borderline cases or statin-intolerant individuals, consider: 

  • Ezetimibe (does not carry the same results with regards to reduced cardiovascular events/risks) 
  • Lifestyle modifications: Omega-3s, dietary adjustments 
  • Natural supplements (e.g. red yeast rice which must be used cautiously) 
  • PCSK9 inhibitors (injectables, very potent and most are unlikely to qualify) 

Bottom Line: Should You Take a Statin? 

If you’ve been advised to take a statin, don’t reject it based on gym myths or hearsay. Work with a doctor who understands both your sport and your physiology. Statins can be a vital tool in protecting your long-term health—especially if you’re using anabolic agents, have elevated cholesterol, or have a family history of heart disease.
